-   GSIS ID
-   Company ID

     
    Photocopy of any of the following Utility Billing Statements:
-   Electric
-   Telephone
-   Water
-   Cable 		 
    Photocopy of US Job Offer sheet
    Photocopy of Visa
    Photocopy of Medical Examination
    Photocopy of NSO Certified Birth Certificate/Birth Certificate
      issued by the local registrar
    Special Power of Attorney (SPA)

    Completely filled-out Application - same form as teacher
    Special power of Attorney (SPA) - same form as teacher
    Recent 2x2 colored picture (1pc.) with signature at the back
    Recent original Certificate of Employment (if employed)
    Photocopy of one (1) valid ID (same options as above)
